Hours Per Patient Day (HPPD)

A skilled nursing staffing productivity metric measuring total direct-care staff hours (often broken out by RN, LPN, and CNA) divided by total resident days, used both operationally and by regulators to benchmark staffing adequacy.

HPPD is the granular operating metric that sits behind both the CMS Five-Star staffing rating and the federal minimum staffing standard, and it is typically tracked separately by staff category because the mix matters as much as the total -- a facility can hit an aggregate HPPD target while being critically short on RN coverage specifically, which is clinically and regulatorily significant since certain services require RN presence. Operators and their landlords or lenders monitor HPPD trend closely because a declining trend, especially in RN HPPD, is one of the earliest indicators of staffing distress, often preceding both survey deficiencies and financial coverage deterioration by one or more quarters.
